If the patient is unable to partake in the discussion because his/her illness has become incapacitating, then the family and friends can decide what happens next. This is a lot of hard work, and it can become frustrating and stressful. More than likely, not everyone will come to terms with what is in the best interest of the patient. However, concentrating on the patients’ best interests and not of anyone else will narrow down your opinions. This is just one more reason why we stress the importance of an advance directive be taken into consideration before the patient is unable to make those decisions.
